✔️ Strategies to understand client’s needs & priorities + how to translate these into tangible goals
✔️ How to develop & document person-centred care plans
✔️ Reviewing care plans efficiently & effectively
✔️ An introduction to organisational systems, tools & templates that support effective GDCP.
Who’s it for
CHSP-funded aged care staff who are actively involved in completing assessments and/or developing goal directed care plans with their clients. Team leaders, coordinators & managers who directly oversee staff responsible for GDCP are also encouraged to attend.
