Aboriginal and Torres Strait Islander Aged Care Assessment Organisations

A small number of Aboriginal and Torres Strait Islander organisations are delivering culturally safe, trauma-aware aged care assessments. This is part of a pilot to better support Elders and older Aboriginal and Torres Strait Islander people to access aged care services that meet their needs.

Advance Care Planning: Taking care of Dying Time so you can get on with living

This culturally sensitive booklet was created to support First Nations communities in planning for future health care. With strong community input and thoughtful design, it helps older people – and those who support them – start important conversations about their values, wishes and end-of-life care.

The booklet guides users through choosing a Medical Treatment Decision Maker, recording personal beliefs, and completing a legal Advance Care Directive.

While designed with First Nations communities in mind, the clear, easy-to-follow format makes it a valuable resource for anyone wanting to plan ahead and ensure their voice is heard if they become too unwell to speak for themselves.

Elder Care Support program

The Elder Care Support program will build a workforce to help older First Nations people, their families and carers, to access aged care services to meet their physical and cultural needs.

Rural, Remote and First Nations Aged Care Service Development Assistance Panel (SDAP)

SDAP provides free professional support to aged care service providers located in rural and remote areas or who provide care to First Nations peoples. It aims to help providers in 3 main areas – provider capacity and support, sector development and infrastructure project management.
